Obesidad y trasplante hepático: La necesidad de incorporar un manejo diferencial

Resumen

Introducción. La obesidad es un desafío significativo en el contexto del trasplante hepático, debido a su impacto en la progresión de enfermedades hepáticas y a las complicaciones asociadas al procedimiento. Es necesario estudiar la relación entre la obesidad, la enfermedad del hígado graso no alcohólico (NAFLD), la esteatohepatitis no alcohólica (NASH) y el trasplante hepático, analizando las complicaciones pre, intra y postoperatorias asociadas en esta población y plantear qué estrategias actuales pueden ayudar a disminuir la morbilidad del trasplante hepático en pacientes con obesidad.

Métodos. Estudio de análisis crítico sobre la relación entre obesidad, enfermedad hepática y trasplante hepático, identificando y reflexionando en los principales desafíos a los que se enfrenta, profundizando en las complicaciones operatorias, las estrategias e investigaciones futuras a realizar.

Resultados. La obesidad es un factor de riesgo principal para la enfermedad hepática grasa no alcohólica, que puede llevar a cirrosis y requerimiento de trasplante hepático. Los pacientes obesos con trasplante enfrentan mayores riesgos y desafíos. Esto plantea interrogantes sobre la selección de candidatos, tratamientos pre y post trasplante, y la asignación justa de órganos. Se necesitan más investigaciones para prevenir la obesidad, desarrollar nuevas terapias y mejorar los resultados en estos pacientes.

Conclusión. La obesidad representa un desafío significativo en el contexto del trasplante hepático, no solo por su impacto en la enfermedad hepática, sino también por las complicaciones asociadas al procedimiento. Abordar este problema de manera integral puede mejorar la supervivencia, la calidad de vida y la optimización de los recursos en salud.
