Características clínicas y análisis integral de costos directos médicos y no médicos previos al trasplante renal en Colombia

Resumen

Introducción. En Colombia se dispone de poca información sobre los costos de la valoración y preparación previas al trasplante renal. El objetivo de este estudio fue estimar los costos desde la perspectiva del prestador.

Métodos. Estudio retrospectivo en adultos trasplantados en 2024. Se integraron costos del receptor y del donante; las bases se depuraron y las variables se describieron y compararon según su distribución. Se modeló el costo directo total con ajuste multivariable y estabilidad por remuestreo (1000 réplicas). Se analizó desde el inicio de la valoración hasta el trasplante (2015–2024), con microcosteo de cuentas administrativas e imputación de costos del donante al receptor, solo costos directos médicos y no médicos, en pesos colombianos de 2024 deflactados.

Resultados. La mediana del costo total fue mayor (p<0,001) en donante vivo/fallecido: 22,7 vs. 15,5 millones; laboratorios/imágenes: 4,6 vs. 3,3; medicamentos/insumos: 3,6 vs. 0,063. Hospitalización/procedimientos: mediana 0 en ambos; apoyos logísticos: 8,3 vs. 5,0 (p=0,20). En donante fallecido hubo mayor edad (51 vs. 38 años), más tiempo en diálisis (76 vs. 20 meses) y en lista (1.507 vs. 68 días); en donante vivo hubo más trasplante anticipado (30 % vs. 15 %) y menos hemodiálisis previa (26 % vs. 44 %). La regresión estimó mayor costo con donante vivo (+8,61 millones; IC95% 4,72–12,5) y menor con régimen contributivo (–4,21; IC95% –7,64 –0,78).

Conclusiones. En esta cohorte, los costos previos al trasplante difirieron por tipo de donante y régimen de aseguramiento y se acompañaron de perfiles clínicos distintos.
