Quiste del conducto cístico: No existe un tal quiste de colédoco tipo VI. Reporte de caso

Introducción. El quiste de colédoco es una dilatación congénita en cualquier parte del árbol biliar, que conlleva un mayor riesgo de formación de cálculos, colangitis, pancreatitis biliar y ruptura; además, es descrita como una patología premaligna. Desde hace 40 años se informa que la dilatación congénita que ocurre en el conducto cístico es una condición bastante rara y la propuesta general parece ser incluirla en la clasificación de quistes de colédoco de Todani como un quiste tipo VI.
Caso clínico. Se presenta el caso de una paciente de 15 años con un cuadro de diez meses de dolor abdominal postprandial, localizado en epigastrio e hipocondrio derecho, recurrente (3 a 4 veces por semana).
Resultado. Se hizo el diagnóstico de quiste del conducto cístico como hallazgo operatorio. Se logró un tratamiento exitoso sin tener que abordar la vía biliar común. El reporte de patología descartó malignidad en la pieza quirúrgica.
Conclusiones. El quiste del conducto cístico es una condición congénita rara, que plantea un reto diagnóstico. El tratamiento es quirúrgico. Sostenemos, de acuerdo con la evidencia más reciente sobre esta condición, que no es necesario una categoría adicional en la clasificación de Todani.
