Criterios de selección y personalización para el uso de cirugía bariátrica y medicamentos en adultos con obesidad: Una revisión sistemática

Introducción. La obesidad es un problema de salud pública a nivel mundial, que se relaciona con enfermedades como la diabetes mellitus tipo 2 y los trastornos cardiovasculares. Las opciones más efectivas para su manejo son la cirugía bariátrica y los medicamentos, aunque la adaptación individual del tratamiento aún está en desarrollo.
Métodos. Se realizó una revisión sistemática siguiendo las pautas PRISMA, en la que se incluyeron 21 estudios publicados hasta noviembre de 2024. Se analizaron los criterios de selección, la eficacia y los riesgos de cada intervención. Los principales resultados evaluados fueron la pérdida de peso igual o superior al 5 % y las mejoras en los indicadores metabólicos.
Resultados. La cirugía resultó más efectiva en casos de obesidad severa, logrando la remisión de la diabetes mellitus tipo 2 hasta en el 60 % de los pacientes. Los medicamentos, especialmente los agonistas del receptor GLP-1, fueron útiles en personas con obesidad moderada o que no eran candidatas a cirugía. La elección del tratamiento se basó en el perfil metabólico, las preferencias del paciente y algunos biomarcadores.
Conclusiones. La cirugía bariátrica ofrece mayores beneficios en obesidad severa, mientras que los medicamentos son una buena alternativa en casos moderados. Aún se necesitan más estudios que analicen biomarcadores y promuevan la toma de decisiones compartida entre médicos y pacientes.
