Eficacia y seguridad del tacrolimus en monoterapia a dosis bajas en trasplante hepático: Experiencia en un hospital nivel IV de Cali

Resumen

Introducción. El tacrolimus es la base de la inmunosupresión en el trasplante hepático, pero su uso sostenido se asocia con efectos adversos dosis-dependientes como nefrotoxicidad, hipertensión y diabetes de novo. Esto ha motivado estrategias de minimización inmunosupresora, como la monoterapia a dosis bajas, buscando mantener la eficacia con menor toxicidad.

Métodos. Estudio observacional retrospectivo realizado en la Clínica Imbanaco en Cali, Colombia, entre abril de 2018 y mayo de 2024. Se incluyeron los pacientes con trasplante de hígado, clasificados según el esquema inmunosupresor como: tacrolimus en monoterapia a niveles bajos (<10 ng/ml) o terapia dual con micofenolato. Se evaluaron variables clínicas, quirúrgicas e inmunológicas, incluyendo complicaciones metabólicas, rechazo agudo, función hepática, infecciones y sobrevida al año.

Resultados. Se evaluaron 119 pacientes. El 86,6 % recibió tacrolimus en monoterapia. Niveles entre 5 y 10 ng/ml se asociaron con menor rechazo (8,7 %), adecuada función hepática y baja tasa de infecciones. Niveles <5 ng/ml se relacionaron con mayor rechazo (21,1 %) e infecciones (13,5 %), mientras que niveles >15 ng/ml se asociaron con disfunción hepática. En comparación con la terapia dual, la monoterapia mostró menor incidencia de insuficiencia renal, hipertensión y diabetes de novo, sin diferencias en la sobrevida del paciente ni del injerto.

Conclusiones. La monoterapia con tacrolimus entre 5 y 10 ng/ml en esta población mostró ser segura y eficaz en pacientes seleccionados, permitiendo reducir la toxicidad sin comprometer la función hepática, la respuesta inmunológica ni la sobrevida.
